Applications to conduct Food Service, Food Handler, Mobile Food Service and Vending Machines are mailed out annually and are due no later than March 1 of that year at the Division of License and Assessments. An application can be obtained by contacing our office at (216) 664-2106 or (216) 664-4374 to receive information specific to your operation.
An application can be obtained by calling (216) 664-2175. Upon completion mail all required documents along with application to our offices for review.
